I reached out to UCTRONICS support, who indicated any RJ45 cables with "aluminum foil shielding" are not recommended. These type of RJ45 cables seem to be fairly popular (a quick search on Amazon shows most premade cables are in fact shielded like this, all of mine included). Since I don't currently have any non-shielded cables, I was able to sort of snip some of the foil from the connection end going into the Raspberry Pi (pictures attached to show what I did). This worked! I assume the foil somehow shorts the power circuit going into the Raspberry Pi or something like that, not too sure. To me this feels like a shortcoming in the design of the Raspberry Pi, but I am not taking any stars off of the PoE Hat as I don't feel it is to blame.
I have a feeling this could affect many others (and mistakenly think their PoE Hat is DOA) - so double check which RJ45 cable you are using!

The bad news is that they will eventually just fail, or fail to power back up. So far it doesn't seem that any have fried a Raspberry Pi, just themselves, but over 2 years I have about a 50% failure rate. The most recent failure was after 13 months.
Update: Customer service reached out with a discount code for replacement. Great support!
And you should because as others have noted this runs hot. The smaller version does fit around CPU heatsinks, but in order to seat it fully you'll need to remove any heatsinks you might have on the ram & ethernet chips - if you are using a decent vented case with a fan this won't matter.
Last note is this is rated for 2.5a whilst the Pi 4 B rates a 3a power supply. This generally only matters if you fully load up the USB ports; if you are planning to use this & your Pi a POE powered network appliance (PiHole, HASS, etc.) you'll be fine, you'll barely top 1.6a in that use case.

If you are not doing alot of processing with your Pi this is the POE hat for you. If you are going to create heat look for another one with a fan but not this ones big brother till they decide to post in the manual how to use the fan control.
Very high failure rate noticed with this product in our use case, would recommend alternate product.

Hat einen Raspberry Pi 3B+ versorgt.
Der HAT lief so heiß, dass man ihn nicht anfassen konnte.
Amazon Support war aber sehr freundlich bei der Reklamation.
Works fine first try on raspi 4.
Most case will need minor adjustment to work ( mine had to remove a plastic layer and put fan on the outside for example )
