I was hesitant using a wrist cuff as opposed to the upper-arm cuff for a variety of reasons including that in both research and in medical settings the upper-arm cuff is just most common. As mentioned, my methods would not have allowed for an upper-arm cuff, so I had no choice. Upon receiving the cuff (I ordered two of them), I first checked for the general reliability of the measure by taking several measures within a few minutes of each other and comparing them. I did the same with each of my research assistants. With a caveat mentioned below, the findings were quite consistent with only slight variability between measures.
The cuff could be reliable from measure to measure without being valid. I did not need medical-level precision, but I wanted something pretty close. To examine this, I took the cuff to a nurse friend of mine and had her manually measure my blood pressure and then we compared those findings to the Omron 7 Series wrist cuff. While not perfect, the measures were close enough that I am confident with the results. It is just as plausible to think that manual measures are not perfect than the cuff, and they were both close.
Finally, I used the cuff in an experimental setting. Without going into details, participants in the experiment were put into a stressful situation and their blood pressure compared mere minutes apart. Those in the stress condition should have seen a significant increase in blood pressure if accurately measured, while those in the control group should not no substantial change. Demonstrating a stress response (i.e., increased blood pressure) was my main reason for using this cuff, and it worked just as expected. The task I used is known for elevating blood pressure in the vast majority of those who undertake it, so a cuff that failed to demonstrate that would have been flawed OR my participants would have been mutants unable to experience stress. The latter would be pretty awesome, but that did not happen. The monitor worked just as I expected, across multiple experiments and multiple participants.
Some considerations:
1. I used this thing a lot. We used this with nearly 300 participants at 4-5 measures per participant. I had to change the batteries out once, but that is to be expected. What impressed me is that this cuff remained operational and reliable across so many measurements of different people. The monitor is made for a home setting, yet it worked just fine in a laboratory setting. Please note that this was not a medical study, and I am not a medical doctor.
2. It works on a wide range of wrist sizes. We had participants who were thin, average, and overweight involved in the study. I recall only one instance where the cuff did not fit someone (the person's wrists were too small).
3. There is a slight learning curve, and you have to use it correctly. This cuff is easy to use, but you have to follow the directions including how to hold your hand. It will not start until your hand is in the right position. If the cuff is on the bones too much, it is not going to work. Once you get used to putting it on this will not be a problem. However, expect a few errors either with aberrant readings or just failed readings until you master using this thing. We are not talking about weeks of practice here--just a day or two.
I am not all that comfortable with the pulse readings, but I will admit that I did not independently verify the accuracy of it because it was not something that was all that important to what I was doing.
Now that my research is complete, I still use this at home just for day to day tracking of my blood pressure. It still maintains its reliability, and I can still do small things that I know will influence my pressure (e.g., measuring immediately after exercise, after coming in from the cold, etc.). It works fine. For home use, I really don't think I would go back to using an upper-arm cuff.
Update: just purchased this new one 652N (which is this flatter looking one, it's a pretty simple device. It comes with 2 AAA batteries needed to operate this, and the battery panel comes off easily from the back. The bottom unit has 2 buttons, select the left button and the date starts to blink, the right button changes the month, day, year, progressing it by the right button that shows an triangle that points to the right on the button. Once you select the right date, month, year, just select the left button again to progress it to the next option and then on to the time, again changing time with the right button, then hitting left button to progress to minutes, then pressing right button to change the minutes. You'll get what i mean once you play with it. The thing i like about this BP monitor is it's super quiet, and it doesn't inflate so rock hard, it seems to inflate softly and still gets an accurate reading, which is nice. The old one gets your blood pressure going a little more when it keeps inflating making you worry you're blood pressure is way up lol. The only thing i dont like? It's the wrist band, i have small wrists and this band can almost fit around my bicep lol, well it's not that big, but it just looks huge for a wrist band. Plus the shape of the band makes it hard to fold back into the small plastic casse, but i do like the size, it's much smaller then old wrist band and less rigid so it fits in this smaller flatter case. So great redesign of something that worked well before, so 5 stars still. Oh and it comes with 5yr warranty so register at www.omronhealthcare.com

The readings correlate quite well with a Boots arm BP monitor and the specification is for +/-2%, which given the variability of BP depending on how stressed you, are is insignificant. The packaging claims it is the number 1 doctor recommended brand and to be "clinically proven accurate".
The unit is also silent in use, which is amazing.
