Took me FOREVER to figure out how to orient the top to be horizontal. It's not clear from the product video at all. The answer is that after expanding it out slightly to give yourself some room, pull DOWNWARD on the rough bit just below the head (DON'T pull UPward on the head, as i'd been trying to do for the longest time). This will release the head, allowing you to rotate the head to the horizontal orientation and then allow it to snap back into the tube. Hope this helps someone. (See attached video)

Took me a minute to figure out the locking mechanism to rotate the hammer from parallel to perpendicular, though once I did I like that it is not screw based but just a pulling lock mechanism. I don't like the threading on the bottom piece to pull the brush out (a little difficult to get the threads lined up).
Absolutely recommend for someone who wants a quality reflex hammer.
1. it was lighter
2. it was made with a different metal that was not as polished or sleek looking
3. the telescoping feature felt crude and even started scratching the metal after opening and closing it a few times
4. the head-hammer interface was noticeably loose
5. when laid on its head flat with the handle fully extended, my hammer would topple while my attendings hammer wouldn't even wobble
I immediately returned the hammer and ordered one from the AAN. I think even if you can't become a member, the hammer will cost $50 but is still better than the one sold on amazon. Maybe you could ask a collegue who is/can be a member to buy you one using their account but your delivery address and card info, considering its the same price exact price, but an entirely different product.
Top reviews from other countries
The cheaper/free hammers aren't weighty or precise enough to elicit reflexes in the patients who deviate from the norm. The harder plastics can also cause pain for some patients during tests.
Other collapsible Babinskis have cheap rubber and even cheaper collapsing mechanisms without a folding head.
But this MDF model is backed by a fantastic warranty and is extremely sturdy. The fact that the head folds flat with the collapsing rod means you can store it like a pen. The extra weight is worth it for the reliability and not looking like a goober with a two foot stick poking out of your coat. Most importantly, the higher quality rubber causes the least amount of discomfort for the patient.
The only caveat is that other doctors will try to steal it from you. But that's true for anything you carry that's under 2 lbs.
