I was also surprised that is is only a 3/8" drive. Should be 1/2" for something you need to use this much force with.
The teeth on the gear don't get a good grip on the drain. My drain had corrosion and gunk (like most do that will have a need for this type of tool) that caused the tool to not sit flat which allowed it to slip. If you are to the point that you need a tool like this you don't care if it chews up the drain as long as it gets removed so the teeth need to be more aggressive.
I was also surprised that is is only a 3/8" drive. Should be 1/2" for something you need to use this much force with.

It slipped frequently and did not stay level so I had to maintain the placement of the tool with one hand and try to turn the ratchet with the other. It was a bit of a challenge but it finally came out.
